Duties and Responsibilities
- Provide nursing assessment of Patients after admission
- Provide and coordinate care by assessing patients’ physical and behavioral health needs.
- Documents information about Triage, Behavioral Health and Physical Health issues.
- Provides direction for behavioral health observation/stabilization services in the crisis area
- Responsible to observe a Patient’s behavior and document such behaviors in a nursing note
- Maintain medical records and educate patients and their families about various physical and behavioral health conditions.
- Record medication brought in by patient on medication reconciliation/storage form.
- Provides education concerning Psychiatric diagnosis, treatment options and psychopharmacology to Patient.
- Initiate documentation and debriefing with Patients for any seclusion/restraint incidents
- Intervenes with appropriate verbal interventions to diffuse out of control behaviors.
- Accurately transcribes Provider orders and administers medications as prescribed.
- Have Patient sign medication consent form
- Performs chart checks and signs off as a second noted on medications prescribed
- Draw labs and provide wound care as ordered by Providers.
- Complete nurse to nurse during shift or when accepting Patient from another facility
- Notify physicians of significant changes in status such as difficulties with detox, medical emergency, change in medical status regarding response to medications, and related challenges.
- Ensure that initial nursing treatment plans are completed upon admission and that all nursing treatment plans are specific to assessed needs.
- Accurately administer medications per ordered note.
- Document accurate count of controlled substance medication administration on required documents.
- Complete admission and discharge processing efficiently and accurately as directed
Knowledge, Skills and Abilities
- Knowledge of Crisis Intervention and ability to de-escalate potentially aggressive situations
- Knowledge of Seclusion/Restraint standards, guidelines and techniques;
- Provide education to patient regarding Behavioral Health diagnosis, medications and treatments
- Demonstrates ability to effectively use therapeutic option theory and technique and can describe and demonstrate their roles and responsibilities.
- Check Patient medication record for accuracy and update as needed.
- Must have patience and good communication skills.
- Adhere to a professional code of ethic and confidentiality.
- Embrace and respect culture diversity.
- Recognize the rights of Patients and their families.
- You are a mandated reporter
Education, Experience and Qualifications
- A bachelor’s or master’s degree in nursing, and
- A valid registered nurse’s license issued by the Arizona State Board of Nursing,
- 1 year or more of nursing experience in a psychiatric, mental, behavioral, or substance abuse or crisis intervention facility
- Emergency room or crisis intervention experience
- Familiarity with Court Ordered Treatment process
- Must have a current Level 1 Fingerprint Clearance Card,
- Current CPR and Mental Health 1st Aid certificate
- Current negative TB skin or chest X-ray within the last six months
- Keep license and CEU current.
