Driver's License Revocation Evaluation & Application

We provide Revocation Evaluations to reinstate your driver’s license if it has been revoked in Arizona. Driving license revocation evaluation requires a Qualified Provider to assess why your license was revoked, and whether the driver has completed any required education or treatment.

To schedule an evaluation, please fill out this revocation application packet from, request an Extended Certified MVR from ADOT's Motor Vehicle Division, add a copy of your Photo ID/Driver's License and email them to ernest@newdawnBH.com for next steps.

Driving privileges may be revoked in Arizona for a number of reasons, including but not limited to:

• Failure to stop and help at the scene of a traffic accident
• Drive-by shootings
• Two or more convictions for reckless driving or DUI
• Driving under the influence of drugs or alcohol
• Felonies involving a vehicle
• Perjury or making false statements to the MVD

We provide Evaluation required by ADOT/MVD for the reinstatement of your Revoked Driving License due to Alcohol or Drug related issues. We will electronically submit your application to ADOT/MVD.
